Consenting and authorizing auto-debit from the specified Savings Account to join the Pradhan Mantri Jeevan Jyoti Bima Yojana with HLIC (HDFC Life) for cover under Master Policy no PM000001, subject to correctness of information provided regarding eligibility and receipt of consideration amount. In case, the above mentioned account is closed or does not have adquate balance to deduct the premium on the due date with your Branch office, you will cease to be a member of this scheme from that date and, no liability will be accorded on the Bank or Insurance company.

Cover Stops: At age 55 years if savings account is active and in force.
